A Note on Confidentiality Before You Begin

We deeply appreciate your leadership and the work you do for your church. The questions that follow are designed to help identify areas where guidance or support could be most valuable. I realize some of these questions touch on sensitive areas of leadership, culture, and the overall health of your church.

Please know that your responses are entirely confidential and are used only to help me understand your church’s situation so I can serve you well.

You have two equally secure options for how to proceed:

Complete the form below – This is the fastest way to give me insight before we meet. Your answers will be kept private and stored securely.
Private conversation option – If you prefer not to submit written responses, we can review the same questions together in a confidential meeting (by phone or in person). This conversation will be protected by a signed Non-Disclosure Agreement (NDA), and I will take and store notes securely.
